Why an X-ray is necessary
An x-ray of the foot is prescribed by a specialist, usually a trauma surgeon or orthopedist, but sometimes a general practitioner can refer a patient for this procedure. He or she will be guided by the patient's complaints, such as: e.g.:
- pain in foot or ankle;
- redness and blue discoloration of the skin;
- cramps in the calf muscles;
- changes in foot and ankle shape;
- swelling of the ankle and foot;
- Restriction and impairment of the mobility of the joint.
These symptoms may indicate an injury or illness. On an x-ray, the doctor can look at all the components of the foot and identify any abnormalities in their structure or position.
In other words, an X-ray of the foot can reveal:
- Foot deformity as a result of an inflammatory disease: arthrosis, gout, arthritis;
- a valgus deformity of the big toe;
- Traumatic damage to the joint and surrounding tissues, fractures;
- presence of osteoporosis;
- flat feet;
- Congenital bone and cartilage abnormalities;
- clubfoot
- clubfoot and pes cavus;
- heel spur.
In addition, the X-ray shows the presence of type one and two aseptic necrosis. The former is manifested by necrosis of the navicular bone, the latter by changes in the heads of the metatarsal bones.
Pathological processes caused by systemic diseases such as diabetes or rheumatoid arthritis also manifest themselves in the tissue structure of the foot. They are therefore clearly visible on X-rays.
X-ray examinations are also prescribed in children for the prevention of various pathologies. With congenital lesions, the doctor can make a preliminary diagnosis, based on which he will recommend further treatment.
Stages and types of treatment
X-rays of the foot in two projections are performed in a special room with appropriate equipment. The patient does not need to make any preparations, he only has to appear at the facility at the agreed time. The patient then approaches the device and removes their shoes, clothing and any foreign objects on their right or left foot.
The doctor's assistant instructs the patient to position herself to irradiate the area to be examined and covers the genitals and breasts with a special apron to protect the patient from the X-ray radiation. The patient is asked to rest for a few minutes; during this time an image of the joint is taken.
The recording can be done in the usual way, with the patient lying on the table in the desired position. For example, in a straight projection, the patient lies supine, bends their knees, and places both feet on the table. With an oblique projection, the patient lies on his back, but the leg is not on the table, but is slightly rotated, usually by 45 degrees, but the angle can be different.
In lateral projection, the patient is placed sideways on the table with the sound leg straight and the affected leg slightly bent at the knee and placed on the table surface. The dorsal-plantar projection is also performed in the supine position. The patient is in a supinated position with the legs straight and the foot bent at 90 degrees. To prevent the foot from sagging, it is fixed with special straps.
There is another way to take a lateral x-ray - an x-ray of the foot under weight bearing. This can be used to determine a transverse flat foot. The essence of this technique is that the patient must transfer the load to the painful leg. The patient stands on it and puts pressure on the other leg, which in turn puts pressure on the foot and reveals any pathological processes in the foot.
For a more accurate diagnosis, doctors usually prescribe 2 or 3 projections at once, one of them with a load. This is because the changes can be subtle and not visible on a single scan.
