Our feet are exposed to very high loads. The load on the feet often exceeds our body weight, for example when running or jumping. In addition to their support and movement function, these body parts must constantly adapt to the terrain, speed and type of walking, absorb shock and keep the entire skeleton safe. But even with such loads, the feet must maintain their function and serve as a stable support, adapt to the ground and cushion the weight. It is not only the most important, but also an extremely sensitive body structure. The complex, anatomical structure of the foot is tasked with adjusting body position and maintaining balance quickly and effortlessly. It is also a shock absorber that protects the internal organs and spine from shocks. Pelvic tilt, posture problems and somatic dysfunctions have a negative effect on the entire body and especially on the musculoskeletal system. If the balance is disturbed, the loads are distributed unevenly on the feet and joints and ligaments are overloaded. This leads to arch imbalance, increased fatigue, pain, valgus deformity and various other disorders.

Sufferers cannot walk for a long time, and if they stand for a long time, the pain becomes sharp and stabbing.
Deformities can appear in childhood but are most common after the age of 30. Women are 4-5 times more likely to suffer from this problem than men. Deformities can be congenital or acquired. The former occur in infants with intrauterine fetal abnormalities or as a result of birth trauma. Acquired curvature of the big toe is caused by various diseases of the spine, injuries to the muscles, ligaments and joints of the leg.

Causes of foot and ankle pain
Foot pain can have many different causes - perhaps, as with diaphragmatic problems, it is not the feet that are the problem. The most common causes of foot pain include the following conditions:
- Tendonitis - inflammation of the Achilles tendonTendonitis is caused by leg overload, walking in uncomfortable and unstable shoes, insufficient warm-up before exercise, obesity and an inactive lifestyle. Symptoms include stiffness and pain in the area of the tendon, and swelling that can be felt on palpation.
- rupture of the tendonTendon rupture is most common in athletes. When it tears, there is a sound similar to a taut rubber band breaking, severe pain, and difficulty standing on tiptoe. Although the Achilles tendon is incredibly strong (with a tensile strength of up to 350 kg), it is the most difficult tendon to injure. In the event of a tear, surgical repair is recommended.
- heel spur – A thickening of the heel bone, which in most cases does not cause any discomfort, but in some cases it can cause pain when running and jumping. Heel spurs develop as a result of intense physical activity. They are characterized by pain in the morning when getting up and moving, and after prolonged sitting. The disease is treated with medication, physiotherapy and osteopathy, wearing orthopedic shoes and special insoles is recommended and, in particularly severe cases, surgery is recommended.
- Osteoarthritis of the foot and its complications – Hallux rigidus – a degenerative disease characteristic of patients over 50 years of age. Over time or due to trauma, the articular cartilage wears away and no longer protects the bones from contact. Symptoms include swelling, pain, stiffness, a deformed foot, and limited mobility. 'Stiff toe' is a complication and consequence of osteoarthritis when the disease affects the big toe joint and the pain is localized there. Osteoarthritis can be treated with a healthy lifestyle and physical therapy, wearing orthopedic shoes, and surgical interventions - endoprostheses, removal of inflamed tissue and spurs and parts of the joint.
How to keep your feet and ankles healthy
Many diseases are preventable and quite simple:
- wear good shoes,
- warm up properly before training
- wear good shoes, warm up before exercise, lead a healthy lifestyle,
- Do simple ankle exercises, especially if you work in a seated position.
The simplest foot and ankle warm-up exercise is as follows:
- Sit in a chair and stretch your legs out so your feet are off the floor.
- Extend the big toe forward.

- Repeat the exercise with the other foot.

The body is a system
Even a small valgus deformity can quickly lead to signs of fatigue and eventually to pain. If the disease progresses without medical care and treatment, there are serious changes in the gait pattern, with the pathological process affecting the spine and large joints.
In severe cases, there can be serious consequences: arthritis, arthrosis, chronic bursitis (capsulitis), which restricts mobility. Dry calluses and corns form on the soles of the feet, causing severe pain when walking.

They are recommended for severe valgus foot deformities and other pathologies. Surgical procedures can affect the soft tissues of the feet, the bones, or both at the same time.
Massage, manual therapy, physical therapy and shock wave therapy, acting only locally on the foot itself, its joints, muscles and sole aponeurosis, have a temporary effect, and such treatment cannot be called complete.
Such methods of treating any type of flatfoot with its complication of a valgus lesion of the 1st toe or pes cavus have no effect on the root cause of the problem, since a holistic approach to the body is excluded. The most important historical assumption of medicine as a science and art of healing - 'Not the disease, but the patient must be treated' - is violated.

Overloading and fixation of the dura, for example due to whiplash, craniocerebral trauma or birth trauma, also play a role. All of these problems affect the biomechanics of the entire body and the feet in particular.
In order to eliminate deformities, it is necessary to address all the pathological connections and limitations of movement that exist in the body of a particular patient, taking into account the individual history and characteristics of the body. In this case, osteopathy is the most effective treatment method.
Methods used by the osteopath
The methods used in osteopathy can be divided into direct (also called structural) and indirect (also called functional).
- As the name suggests, direct techniques work directly on a tissue or organ to improve mobility, normalize tension or improve venous and lymphatic drainage.
- With the indirect techniques, rigidity of movement is avoided by relaxing the tissue as much as possible. The mechanoreceptors are reprogrammed and the periarticular soft tissues relax without any manipulation.
COMPLICATIONS OF THE EQUINUS FOOT
In children, the equinus deformity is often combined with a transverse flatfoot due to a malposition of the foot. The child shifts body weight to the first half of the foot and the arch gradually lowers while the heel remains elevated.
When equinus is associated with transverse flatfoot in children, treatment should primarily focus on the root cause, the equinus deformity.
Left untreated, it causes longitudinal and transverse flatfoot in children – the foot flattens out both lengthwise and widthwise, but the main symptoms appear in patients over 45 years of age.
Diagnosis of equinus deformity
An experienced podiatrist usually only needs a visual exam and a series of movement tests to raise suspicion of a foot deformity. In addition to the equinus deformity, however, an X-ray can also be helpful. The doctor can see the position of the bones and measure the range of motion of the foot and joint flexion angles. X-rays help in deciding on treatment options and in assessing the course of the disease.
In some cases, computed tomography (CT) is used for more detailed imaging. The orthopedic trauma surgeon creates a series of images that can be viewed on a computer screen. A three-dimensional model of the leg skeleton is created and rotated in different directions. CT scans are used in complex cases and in preparation for surgery to determine the best type of surgical procedure.

The importance of osteopathic treatment of children
The development of flat feet in children can often be traced back to birth. The connection is direct, although not obvious in the medical community. When a baby passes through the birth canal or is forcibly pulled out during a cesarean section, the occiput often protrudes—it bumps into the spine. The body is unable to restore its natural balance and begins to adapt. The shoulder blades arch, the lower back falls inward, the weight of the body presses on and deforms the arches of the feet, the child develops clubfeet.
