DEPARTMENTS
The Department of Traumatology and Orthopedics was founded more than 60 years ago. Over the years, the founders of orthopedics and traumatology, well-known professors and scientists, who later became heads of specialized institutes in our country, worked here. The experience gained during this period, as well as advanced technologies and methods, allow us today to offer highly qualified care at the most modern level and to achieve good and excellent results in the treatment of children with congenital and acquired diseases of the musculoskeletal system.
In 2018, the department was named after Professor VP Nemsadze, one of the founders of pediatric traumatology and orthopedics, who worked in the department for 47 years.
The department has 60 beds, including 30 trauma beds, 20 orthopedic beds and 10 neurotrauma beds for children. The staff on the wards are based on the age of the children in order to offer them as much comfort as possible. There are also separate cabins for small children, where they are accommodated with their mothers.
More than 3,000 reconstructive operations of varying complexity are performed on children and adolescents with the following diseases every year:
- Congenital anomalies and malformations of the musculoskeletal system (syndactyly and polydactyly, polyphalangy, ectro- and clinodactyly, deformities and shortening of the fingers, shortening of the metacarpal and metatarsal bones, shortening and deformity of the limbs, clubfoot, ankle defects, club hand, amniotomy, false tibia joint, hips; Congenital torticollis, incorrect clavicle joint, Sprengel disease (high shoulder), congenital hip dislocation and displacement, valgus and labral deformities of the proximal thigh (coxa vara, coxa valga).
- Traumatic injuries and long-term consequences (closed and open fractures of any complexity and location, fractures of the hand and foot, combined and multiple injuries, intra-articular injuries and their consequences, fractures in infants and young children, abnormally healed fractures, incorrect joints, post-traumatic deformities and limb shortening , fingers and feet).
- Diseases, injuries and consequences of injuries to large joints (cruciate ligament rupture, meniscus, luxating patella, osteochondropathies of the knee joint (Kenig's disease, Levene's disease, etc.).
- Musculoskeletal disorders (scoliosis), aseptic necrosis of the femoral head (Perthes disease), localized epiphyseal dysplasia (juvenile epiphysiolysis, Blount disease, humerus varus), stenotic ligamentitis (Nott disease)
- Foot malpositions in children and adolescents of different ages - the most modern surgical treatment methods are used here, including reconstructive surgery with shortening and relocation of the tibia and foot muscle tendons and arthrodesis of the subtalar joint with modern implants.

Who are the Referrers?
I worked in the Office of the Presidential Administration for a year. After #Medvedev became Prime Minister, we moved to the department for the preparation of texts for public speeches of the Prime Minister of the Russian Federation. I worked there for about four years.
The entire presidential administration is divided into departments. And we saw ourselves as a unique structure – we didn't call ourselves Directorate, we called ourselves Referentura. The Referentura takes care of all the President's public appearances, as well as telegrams, congratulatory and welcome letters on his behalf. The structure has existed since the Yeltsin era and is constantly changing, but when I worked there everything was organized quite simply.
At the top are the manager, his deputy, the chief referee and the clerks. They are appointed by the President himself. These are people who have been working in this environment for many years and who write the most important texts. Then come the standard positions – chief consultant, consultant and speaker. They are not hired by the president, but by the head of the board of directors.
In total there are about 15 people in the reference group - from those who write the simplest texts to those who discuss issues with the President. I went there to get an entry-level position, but when you look at all of the administration, including the couriers and typists, I was somewhere in the middle of the career ladder. No one expected me to be serious at the entry level: everyone knew that a freshman wouldn't be able to write anything serious the next year. But in five years I've never written anything serious. I realized pretty quickly that I wasn't interested. So I did everything, but I didn't overexert myself.
The hardest part is working with the head of the reference department. He hardly ever writes himself, but he needs to know what to write to others and in which texts. The head of unit must get a very good sense of the mindset of a public figure. Sometimes it has to be done very quickly. When there is a terrorist attack, the head of unit cannot write: 'How terribly sad this is all', he has to understand how the president or the prime minister himself will react. The job of the head of department is to hit nine out of ten cases, preferably all ten. That's why the President is always looked after by friends - not from the old bathhouse, of course, but by those who worked with him a lot.
Some of the speakers are image bearers. You can advise the President: 'Dmitry Anatolyevich, that would be better'. Of course, the final decision remains with him, but he is not an autocrat and often listens to her.

Telegrams from the President are a creeping impertinence. How can it be that real telegrams are still being sent, that is, a piece of paper with a red card pasted into the envelope. Every day we prepared five or seven telegrams, greetings, congratulations and condolences. It happened that the President sent them back - it was a failure. Once he said: 'That's not the President's level.' At first it was very strange for me to sign my name 'Medvedev' and write on behalf of someone other than myself. You write 'I am this, I am that', but you refer to the third person and to the third person. Anything beyond the reference could be considered a faux pas. A name might have been written into the telegram template – for example 'Konstanty Konstantinovich'. From time to time we could wink, and the salute 'to Konstantin Konstantin' went to the head of the Presidential Administration for approval. But I can't remember a slip of the tongue ever reaching the President. We read the texts one by one to avoid typos. It took us a long time to get the letter 'e' right. One day it worked, then not anymore, then not again.
