The Consultation and Diagnostic Center provides medical care for children from birth to 18 years of age, regardless of their place of residence, including within the framework of territorial compulsory health insurance.

For medical care, a prior appointment according to the schedule of the specialist is required.

When contacting the reception, the legal representative (a parent) presents a full package of documents, and an ambulance card for the child is opened and kept. In the event of a delay, the consultation or examination may be postponed to another time or day if the doctor still has space. The question of further hospitalization is agreed upon with the doctor at the consultation appointment at the CRK (see Regulation on inpatient care).

After providing medical services, the doctor issues the patient (the patient's representative) with a medical report with recommendations and a diagnosis for the medical institution at the place of residence.

Medical care is guaranteed:

  • within the framework of the territorial program of compulsory medical insurance of the city of Moscow in a scheduled form (Medical care at the expense of compulsory health insurance is provided according to the list and scope determined annually by the Commission for the Development of the Territorial Compulsory Health Insurance Program).
  • under voluntary insurance programs with paid benefits from the insurance company with which the child's parents have contracted

MHI

To apply for a medical benefit under the MHI, you must provide the following documentation:

    A referral from the local health authority stating the purpose of the consultation, or a referral for a consultation at a Level 1 (polyclinics) or Level 2 (district consultation and diagnostic centers) healthcare facility on Form 057/-04. The referral must state the purpose of the consultation, the results of the initial examination at the place of residence (summary report) and the preliminary diagnosis;

The referral must be signed by the management of the medical facility and bear the stamp of the facility. Referrals issued by medical institutions are valid for 10 days.

Failure to provide the documents listed above or the absence of any of these documents is grounds for refusal of a free MHI consultation.

VHI

The following documents are required for the use of medical services within the framework of the VHI:

  1. Letter of guarantee from the health insurance company (by fax the day before the visit or examination to the outpatient clinic registration number 8 495 936-91-57) or a direct contract for medical care;
  2. The child's birth certificate or passport;
  3. The child's SNILS;
  4. Passport of the person accompanying the child (parent, guardian, other legal representative). If the accompanying person is not the child's parents, a copy of the parents' document is also required);
  5. Parents' SNILS (if the accompanying person is not the child's parent, a copy of the parents' SNILS must also be provided);
  6. For an attendant who is not the child's parent, a notarized statement/consent to accompany and represent the child at the medical facility.

How to get a referral to a private clinic

Private health centers typically refuse to treat patients free of charge without a referral because they risk not being paid for the services provided by TFOMS.

To obtain a referral to a private clinic, you must contact the public health center in your place of residence or empowerment.

The doctor issues a referral on a prescribed form that identifies the private medical facility to which the patient was referred. It must be signed by the head or chief physician of the polyclinic.

For the doctor, it doesn't matter to which clinic the transfer is made: the only important thing is that it works within the framework of the statutory health insurance system and provides the services required by the patient.

Which services you have to pay extra for

When you visit a private medical facility, you should remember that not all services are free and that you may have to pay extra for some services. You have to pay for all services that are not covered by the statutory health insurance (CMI).

For example, if a woman has a quota for artificial insemination, she needs to have some additional tests done at her own expense. The same applies to dental treatment: While a filling can be covered by statutory health insurance, teeth whitening and crowns have to be paid for by yourself.

Unscrupulous private clinics often take advantage of this fact and actively force additional services on the patient. You show up for a free 15-minute appointment with a specialist and leave with a multi-digit bill for testing.

Step 3: Contact the desired medical center

The last step is to call the hospital to clarify the details:

  • when you can come
  • what documents you need to bring with you;
  • whether any tests need to be performed – the results become out of date very quickly;
  • how many days you have to stay in the hospital or if you can leave it the same day.

Sometimes the hospitals are not able to admit you literally the next day, but can admit you a little later. It's best to find this out before you get a referral - call or write to your chosen hospital and ask when they're ready to admit you with a particular diagnosis.

If you travel to Moscow for treatment, all expenses for accommodation, meals and living in Moscow are borne by the patient himself. For example, if a family takes a relative and accompanies him, the hotel is paid by them; this is not included in the MHI.

Sometimes people feel embarrassed to ask for a referral and travel to hospitals in other regions, but this is normal practice. In Russia, in the system of statutory health insurance, there are inter-territorial settlements - in this case, a hospital operating within the framework of statutory health insurance treats a patient from another region and bills the treatment to the fund in its region. According to the think tank Vademecum, in 2018 such settlements between health insurance funds from 69 regions amounted to RUB 136.5 billion.

The doctors found him in the first year of her life, first with suspicion in the maternity hospital and then during a routine check-up. We have never been to a state clinic, we have always been to a private one, but the doctor at the private clinic could not give us a referral for free treatment in Moscow.

We first went to the Ryazan cardiology outpatient clinic to confirm the diagnosis. The doctor there rebuked us and threatened us: 'You must go to Moscow urgently! Urgent! And why do you go to a private clinic? The doctors there are not responsible for anything. I left her in tears, sure that if we didn't go to Bakulevka tomorrow, the baby would be bad. And so we did, a month later.

Another doctor in the same cardiology department referred me to the Bakulev Cardiovascular Surgery Center in Moscow. And it turned out it wasn't so bad after all. The doctor in Ryazan said our daughter had about two leads and the cardiologist in Bakulevka laughed at that and said it was physically impossible and 'probably your doctor just didn't get enough sleep after the New Year holidays'.

We brought our ultrasound and ECG results to Bakulevka, but it turned out that the cardiologist could not make a decision based on them. The equipment in the Ryazan hospitals is less accurate than in Moscow, and the doctor is unwilling to make a diagnosis or prescribe an operation based on these results. She advised us to take a test, which we had to pay for ourselves.
